How To Order

There are 4 methods for ordering:

1. By telephone. Have payment information available. 801-254-7181
2. By completing the form (click Order Form below) and submitting it on-line.
3. By faxing the completed form and providing payment information. 208-361-9418
4. By completing the form, printing and mailing it via regular mail together with payment and/or payment information.
   
   + Money orders and company or personal checks.
   Purchase orders accepted only for multiple products or licenses.
   
   NOTICE: If mailing or faxing a printed form please insure that all information appears on the printed copy. The length of some email and regular mailing addresses exceed the text box on the form and are cut off when printed. If this applies, please write in the missing information prior to mailing or faxing. There is no problem when submitting online.

If requested via download (order form option), I will provide a link and instructions in an email message to you. The installation is automatic after the download. The manual is placed in the installation folder. Cost = $79.00.

If you are ordering PalmCogo or PocketCogo, either alone or with QuickCogo, please check the appropriate box on the order form. If ordered alone The cost is $49.00. When combined with QuickCogo (now or later) the cost is $24.00. Add $5.00 if program(s) sent on CD via regular mail. Programs will be on the same CD if ordered together. The CD will contain the program files, a letter of instructions and the manual.

Combination of QuickCogo and a PDA program ($103.00)

Additional QuickCogo license ($40.00)

Receive software on CD ($5.00) (Includes receiving download link for immediate access)

Discounted QuickCogo if PDA was previously purchased ($54.00)

Discounted PDA program if QuickCogo was previously purchased ($24.00)

Hourly charge for customization or calculation and mapping services ($20.00)

Other ($1.00) Use for miscellaneous items not listed but described under Comments. Then adjust the quantity at checkout to equal the purchase amount. This can also be used in combination with other listed items to arrive at the correct total.

After each selection you may review the items in your cart and proceed to checkout when done. Your transaction is completed at the PayPal Site. Please use the comment area for other instructions or for the description of what you wish to purchase if it cannot be added to the shopping cart as previously explained.

If you are ordering by mail, enter the item(s) to be purchased and any instructions in the comment box, then copy this form and mail to the address below. DO NOT USE SHOPPING CART.
